In short

  • Most back pain is muscular or joint-related and recovers well with the right treatment and movement
  • Radiating pain into buttock or leg is often sciatica, an irritated nerve
  • Staying moderately active beats bed rest, we show you how
  • Usually seen within one working day, no referral needed

How do you recognise back pain that needs attention?

Almost everyone gets back pain at some point, and in most cases it is not caused by serious damage. Overloaded muscles, stiff joints of the spine and long days of sitting are by far the most common drivers. Still, back pain can seriously limit your work, sport and sleep, and that alone is reason enough to have it looked at.

Typical symptoms we see in the practice:

  • A dull, nagging ache in the lower back, worse after sitting or in the morning
  • A sudden sharp pain after lifting or twisting (acute lumbago, a 'locked' back)
  • Pain radiating into your buttock or leg, sometimes with tingling or numbness (sciatica)
  • Stiffness that makes bending, tying your shoes or getting out of the car difficult
  • Back pain combined with tension between the shoulder blades from desk work

The pattern of your pain tells us a lot. Pain that stays in the back points to muscles and joints; pain that travels below the knee points to a nerve. That distinction shapes the whole treatment plan.

Common causes of lower back pain

In Eindhoven's international community, the causes we see most are remarkably consistent: long hours at a screen, a body that gets less movement than it is built for, and then a sudden peak load, moving house, lifting a suitcase, an enthusiastic first gym session after months off.

  • Prolonged sitting and a static work posture, at the office or at home
  • Lifting with a bent back, or lifting more than your back is currently trained for
  • Reduced core and hip strength, so the back takes over work it should not do alone
  • Stress and poor sleep, which measurably increase muscle tension and pain sensitivity
  • A bulging disc (hernia) or narrowed nerve passage, in a minority of cases

Good to know: scans rarely change the treatment. Research shows that many people without any back pain have disc bulges on an MRI. That is why we treat what we find in the examination room, not what a scan might show.

How long does back pain take to recover?

Acute back pain (lumbago) typically improves substantially within two to six weeks. Sciatica takes longer: most people recover well within six to twelve weeks without surgery. Back pain that has been present for months needs a different, more gradual approach, but also then the prognosis with active treatment is good.

The biggest risk is not the first episode but the pattern: back pain that returns every few months because the underlying cause, strength, movement habits, work setup, was never addressed. Our treatment plans are built to break exactly that pattern.

What can you do yourself?

  • Keep moving: walk several times a day, even five minutes helps more than lying down
  • Avoid prolonged sitting; change position at least every 30 minutes
  • Use heat (shower, heat pack) to relax tense back muscles
  • Keep lifting light loads with a straight back close to your body, do not stop lifting altogether
  • Sleep on your side with a pillow between your knees if lying flat is painful

Complete rest is the one thing research consistently advises against. Your back recovers through controlled movement, and our job is to show you which movement is right for your stage of recovery.

How we treat back pain and sciatica in Eindhoven

Your first appointment (25 minutes) starts with a thorough examination: movement tests, strength tests and, when radiating pain is involved, nerve tests. You get a clear explanation in plain English of what is driving your pain, and treatment starts the same day.

  • Manual therapy to restore movement in stiff spinal joints, by our Master-level manual therapist
  • Dry needling for deep muscle knots in the back and glutes (common with sciatica-like pain from the piriformis muscle)
  • A progressive exercise programme for core and hip strength, tailored to your work and sport
  • Practical load advice: desk setup, lifting technique and building activity back up

A treatment plan is usually 3 to 6 sessions. With sciatica we monitor the nerve symptoms closely, and in the rare case that warning signs appear, we refer you to the right doctor immediately.

When should you see a GP first?

Physiotherapy is directly accessible and the right first step for almost all back pain. See a doctor first, or tell us immediately, if you experience:

  • Loss of control over bladder or bowels, or numbness in the saddle region
  • Rapidly increasing loss of strength in a leg
  • Back pain with fever, unexplained weight loss or after a serious accident
  • Constant night pain that does not change with position

These signs are rare, but they need medical assessment before physiotherapy. We screen for them in every intake.
